Frequently Asked Questions

Biological Dentistry in Orange County & Southern California

What is biological dentistry?

Biological dentistry is a comprehensive approach to oral healthcare that considers the relationship between oral health and overall well-being. At Biological Dentistry of Southern California in Irvine, we prioritize biocompatible materials, advanced diagnostics such as CBCT 3D imaging, and minimally invasive techniques.

Treatment recommendations are based on clinical findings, patient history, and continuing education. Outcomes vary by individual, and no specific health results can be guaranteed.

What conditions do biological dentists evaluate?

A biological dentist may evaluate:

  • Chronic dental infections

  • Infected root-treated teeth

  • Periodontal (gum) inflammation

  • Jaw joint dysfunction (TMJ)

  • Bite instability

  • Residual infection in extraction sites

  • Material compatibility concerns

  • Structural bone defects visible on CBCT imaging

Each diagnosis is based on objective clinical and radiographic findings.

What is SMART mercury removal?

SMART stands for Safe Mercury Amalgam Removal Technique. It is a protocol designed to reduce exposure during the removal of amalgam fillings. This includes specialized suction, isolation techniques, and protective measures.

SMART certification reflects additional training beyond standard dental education. Removal of amalgam is performed only when clinically indicated and after informed consent.

Do you place zirconia (ceramic) dental implants?

Yes. Zirconia dental implants are a metal-free alternative to traditional titanium implants. Treatment recommendations depend on bone quality, anatomy, and overall oral health.

Implants restore function and appearance but do not replicate the exact sensation of natural teeth.

Do you perform root canal treatment?

Our practice does not perform root canal procedures.

When evaluating a tooth with pulpal infection or prior root canal treatment, we provide a comprehensive assessment using clinical examination and CBCT imaging when indicated. Treatment options are reviewed in detail, including risks, benefits, and alternatives.

If a patient chooses to pursue root canal therapy, we refer to qualified endodontic specialists. If a patient prefers alternative treatment options, those are discussed following appropriate diagnosis and informed consent.

All recommendations are individualized and based on clinical findings.

Do you remove root canals?

Biological Dentistry of Southern California is a root canal-free practice, and we do not perform root canal procedures. Our clinical philosophy is grounded in current evidence and ongoing research regarding the potential impact of retained, root-canal–treated teeth on immune function and chronic inflammatory burden.

Conventional root canal therapy is widely used to preserve teeth with pulpal pathology. However, some peer-reviewed research has identified possible associations between root canal–treated teeth and markers of systemic inflammation or microbial persistence. For example:

  • A study in Journal of Endodontics discussed microbial biofilms and persistent bacteria in root canal systems that may resist complete disinfection, noting their potential to maintain low-grade inflammatory states.

  • Research indexed on PubMed has reported association between endodontic lesions and elevated systemic inflammatory markers, suggesting that unresolved endodontic infections may contribute to systemic immune activation in susceptible individuals.

These findings are part of an evolving field of study; causality between root canal therapy and systemic disease has not been conclusively established. Nonetheless, as a biological dental practice, we incorporate these research observations into our diagnostic and treatment philosophy.

When evaluating a tooth that has had root canal therapy or when pulpal infection is diagnosed, we perform a comprehensive clinical assessment, including CBCT imaging when indicated, to determine the status of the tooth and surrounding bone. We then review all appropriate treatment options — including risks, benefits, and alternatives — so that patients can make informed decisions that align with their health goals.

Since our practice does not perform root canal therapy, patients who prefer that treatment option may be referred to a qualified endodontic specialist. All recommendations are individualized and based on clinical findings, diagnostic imaging, and informed consent.

Do you use 3D imaging (CBCT)?

Yes. Cone Beam Computed Tomography (CBCT) allows for three-dimensional imaging of bone structures, sinuses, nerve pathways, and potential pathology that may not be visible on traditional 2D X-rays.

CBCT is used when clinically necessary and in accordance with radiation safety guidelines.

Can dental infections affect overall health?

Oral infections — including deep periodontal disease, chronic periapical inflammation, and microbial biofilms associated with non-vital or compromised teeth — have been the subject of ongoing scientific investigation with regard to systemic health.

While conventional dentistry focuses on local management of infection, some peer-reviewed studies have observed associations between chronic oral inflammatory conditions and systemic inflammatory markers. For example:

  • A review published in the Journal of Clinical Periodontology reported that periodontal infection may be associated with elevated systemic inflammatory biomarkers, and that chronic oral inflammation may contribute to immune system activation in susceptible individuals.

  • Research in Journal of Endodontics described the persistence of complex microbial biofilms in root canal systems and periapical tissues that can resist complete disinfection, noting the potential for ongoing localized inflammation.

  • A study indexed on PubMed investigated endodontic lesions and systemic inflammatory responses, finding that unresolved periapical pathology correlated with increased systemic inflammatory markers in some patients.

These studies contribute to an evolving body of evidence exploring how chronic oral inflammatory states may be associated with systemic inflammation. It is important to emphasize that association does not prove causation, and consensus regarding systemic disease causation remains under investigation.

At Biological Dentistry of Southern California, we consider this scientific context when evaluating chronic oral infections and when forming treatment recommendations. Comprehensive clinical examination, radiographic assessment (including CBCT imaging when indicated), and individualized risk assessment inform our treatment planning.

Our goal is to diagnose and manage oral inflammatory sources based on objective findings and to review potential implications for overall well-being. Treatment decisions are made collaboratively with each patient, with discussion of risks, benefits, and evidence-based alternatives.

Are your materials metal-free?

Yes. At Biological Dentistry of Southern California, we do not use metal materials in treatment.

Our clinical philosophy is based on minimizing potential sources of chronic irritation, galvanic reactions, and material incompatibility within the oral environment. Peer-reviewed literature has documented that dissimilar metals in the mouth can generate electrical currents (oral galvanism) and that corrosion of certain dental alloys may release trace ions over time. While the clinical significance of these findings varies among individuals and remains an area of ongoing research, we incorporate this scientific context into our material selection process.

For that reason, we utilize metal-free alternatives such as:

  • Zirconia ceramic implants

  • Zirconia crowns and bridges

  • BPA-free composite materials

  • Biocompatible ceramic-based restorations

Material recommendations are based on clinical evaluation, structural requirements, and patient-specific considerations. All treatment planning involves informed consent and discussion of available alternatives.

Our commitment is to provide restorative solutions that support structural stability, functional integrity, and material compatibility — without the use of metal in the mouth.

Do you use fluoride?

Our office does not use fluoride products as part of our preventive or restorative protocols.

At Biological Dentistry of Southern California, we focus on individualized preventive strategies that emphasize diet, oral hygiene, structural stability, and minimally invasive care. Preventive recommendations are tailored to each patient’s clinical findings and risk assessment.

Fluoride remains widely used in conventional dentistry. Patients who are considering fluoride-based treatments are encouraged to discuss their options with another dental provider so they can make an informed decision based on their individual needs and preferences.

We are happy to review your concerns and explain our approach during your consultation.

What makes your office different from a conventional dental office?

We emphasize:

  • Advanced diagnostics

  • Comprehensive treatment planning

  • Biocompatible material selection

  • Structured treatment sequencing

  • Continuing education in biological dental protocols

Patients are encouraged to schedule a consultation to determine whether our approach aligns with their goals.

Do you treat TMJ and bite problems?

Yes. Jaw joint function and bite stability are evaluated as part of comprehensive exams. Treatment options may include occlusal adjustment, orthotics, or restorative correction depending on findings.

What are CSR/cavitations?

Cavitations refer to areas of altered bone density that may occur at previous extraction sites, due to covered socket residuum (CSR). These areas are evaluated using CBCT imaging when clinically indicated. Treatment recommendations are based on objective findings and patient symptoms.

How long does treatment take?

Treatment timelines vary depending on complexity. Some patients complete treatment in stages over several months, while others may benefit from more condensed scheduling. A personalized timeline is provided after comprehensive evaluation.

What is the Swiss Efficiency Concept?

Our treatment planning approach organizes care into structured phases to promote efficiency and stability, and to prioritize getting as much work done in as few visits. This has been shown to promote the best scenario for optimal healing.

Do you offer sedation?

Sedation options depend on medical history and procedure complexity. Local anesthesia is standard. For certain procedures, anesthesia services provided by a licensed anesthesiologist may be recommended.

All sedation requires medical review and informed consent.

Do you treat patients from outside Orange County?

Yes. We see patients from throughout Southern California and other regions. Out-of-area patients receive comprehensive diagnostic evaluation and a sequenced treatment plan prior to scheduling major procedures.

Do you see emergency patients?

Yes. Emergency visits include evaluation and diagnostics. Definitive treatment may require comprehensive assessment to ensure proper sequencing and long-term stability.

Do you accept insurance?

We are a fee-for-service office. Patients may submit claims independently for potential reimbursement. Coverage varies by policy and is not guaranteed.

Do you offer financing?

Yes. Third-party financing options are available and discussed during the initial visit.

Do you perform cosmetic dentistry?

Yes. Zirconia crowns, veneers, and implant restorations are available to improve function and aesthetics. Cosmetic procedures are typically recommended after stabilization of active disease.

Do you offer facial esthetic procedures?

In accordance with California dental regulations, we provide PRF (platelet-rich fibrin) procedures derived from the patient’s own blood in areas directly related to oral and perioral structures. These treatments are discussed during consultation to determine appropriateness.

We do not offer Botox or dermal fillers.

What products do you recommend for home care and nutrition?

We provide educational guidance regarding oral hygiene products and nutritional considerations. These suggestions are informational and should be reviewed with a primary healthcare provider before starting any supplement regimen.

How do I schedule an appointment?

Appointments may be requested by phone or through our website. A booking deposit is required to reserve clinical time. Financial policies are reviewed prior to treatment.
