At Biological Dentistry of Southern California we strive to find the right fit between patient and doctor.
Please send us the following documents for your patient application:

Most recent digital panorama x-ray image and any other dental X-Rays (if available

your vitamin D levels (if not available, there will be an additional expense to measure chair side at the comprehensive exam)

International, Out-of-State, and Long Distance Patients
We strive to give the best treatment possible to all patients interested in being accepted at Biological Dentistry of Southern California. For this reason, we allow for potential long distance patients to be provided with a free of charge preliminary and non-binding assessment of their situation, if they are a long distance patient. This assessment includes a visual and written treatment plan, information on each procedure, appointment description and sequence, and cost estimate. We also schedule in-person comprehensive exams to fully evaluate your situation. Upon receiving the assessment, the patient then has the option to book their first appointment of their Swiss Efficiency Concept in which their comprehensive exam, official diagnosis, and treatment plan will be the first part of the appointment if they wish.
By filling out the information below and receiving the free assessment, the patient is allowed to have an in-person comprehensive exam and consultation, and then go straight to the treatments on the same day if they wish. If this is the option chosen, then on the first day of treatment, there will be an official diagnosis and final treatment plan completed and presented after initial CBCT imaging and medical questionnaire is reviewed. This option is best for our fly-in and distance patients, as the first and largest phase of treatment can be planned and completed in one trip.
Of course, an in-person comprehensive exam not on the first day of treatment is always welcomed and has its benefits. After filling out the contact information, we will contact you with the assessment and with further information about how to proceed with treatment or with the in-person comprehensive exam.
